Under what circumstances would you give up this rabbit? (High cost vet bills, digging, destructive chewing, family illness, moulting, biting, moving house, allergic to the rabbit or hay, lack of house training)

How many hours a day will you have time to play with the rabbit?

If your adoption is successful, do you agree to leave Kelly’s Rabbit Warren Rescue Inc as the emergency contact on the microchip registry? (This is if the rabbit goes missing and you are uncontactable, in which case we will be able assist.)

Do you currently have a vet? If so, please provide us their name and phone number. If you do not have a vet please provide two personal references. We reserve the right to contact these referees should the application proceed.

Is there anything else you would like to tell us about you, your family or your interest in adopting a rabbit? The more information you provide, the better we will be able to determine the suitability of your chosen rabbit(s).

Do you consent to a volunteer contacting you to arrange a property check?

Do you agree to have the rabbit health checked once every 12months and vaccinated as required?

Do you agree not to give away or sell your rabbit to a third party?

Do you understand that we will be the secondary contact on the microchip of the rabbit? (This guarantees the safe return of the rabbit.)

By submitting this application, I agree that all the information I have provided is correct. I agree to allow Kelly’s Rabbit Warren Rescue Inc. to check the references I have provided. Providing untruthful answers or failure to comply with the requirements of this application or the signed adoption contract can result in the forfeiture of the adoption

We will require the successful applicant to sign an adoption agreement and allow a property check before adoption. We may also ask to sight your driver licence before the application proceeds. We appreciate your taking the time to complete this application.

We reserve the right to refuse any applicant and will not enter into any further correspondence on our decision.
